As the parent or legal guardian of the above player, I understand that the Elkader Park Board and Elkader Youth Soccer League Committee cannot be held responsible for any injury to this player as a result of their involvement in the game of soccer. I give consent for the emergency medical care prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry should I not be present to make such decisions regarding this child.
